One critique I've heard of hierarchical, vertically integrated systems is that accurate information flow is difficult, based upon the fact that people are fearful for their careers (or even their lives, in extreme situations). So when asked about something they'll say what they feel their superior wants to hear, rather than what may the truth.

This problem can probably be seen in a variety of examples, from modern corporations to the former USSR. I'm interested in what people here think about this issue...
