Yugoslavia was created 1918 and was a monarchy.
1941 to 1944 it was under Nazi Germany (If I remember correctly)
In the middles of 1940's, Tito and his party took control over all the countries and created Socialist Yugoslavia.

My question is; If Tito didn't create Socialist Yugoslavia, then would there be any different land areas today or what would happend to the Yugoslavian part of Germany if the Tito didn't take control?

If it is, is Tito the reason why Croatia, Serbia, Slovenia etc are own countries today?

They would probably be separate states anyway. Yugoslavia was an ethnic-warzone-waiting-to-happen from the very beginning.

For most of its history the Balkans were divided between two imperialist powers. The ottoman empire, and the Austro-Hungarian one.

Only after WW1 was the Kingdom of Yugoslavia created, which didn't really last long since the Nazis attacked us some 2 decades later.

If Tito didn't take power, someone else from the Partisan ranks would have. Seeing that Yugoslavia was a nation which managed to (for the most part) liberate itself. The Peoples Liberation Army was an organized resistance force and carried out one of the most successful guerrilla campaigns in history.

People forget that we weren't liberated by the Allies or Soviets. We were liberated by our very own Partisans. The Partisans fought on three fronts. Against the Germans/Italians, against the Ustasha (Croat Nazis), and against the Chetniks (Serbian Monarchists), and the Partisans won. The Soviets did some fighting for the liberation of Belgrade, however at this point in the war the game was up and Belgrade was going to fall with or without the Soviets.
