As socialists we of course would not acknowledge democracy under the capitalists system to amount to any sort of real freedom.Even with an elected government, as we can see across the world, the vast majority of people in society continue to be exploited every day.
Socialists seek to establish direct grass roots democracy though community councils and in the work place, say to request funding for your local area for roads etc I would like to know in detail how this would work, at what levels would there be represenation,would there be middle-men to prevent abuse of the system, and how it would work day to day in practical terms.

Workers and community members getting together and democraticaly decide on different matters. Of course there should be centralization, ie workers councils should send elected and recallable representatives, to a "nation"-wide excecutive. That would be the cetral power of the government.

There's no needs to re-invent the wheel. Municipal, state/province and "national" levels are practical enough.

No. The "middleman" that prevents the abuse of the system is workers democracy itself.

While democracy (or representative democracy) does exist in bourgeois countries it is heavily flawed by the realities of capitalism. In bourgeois governments only a few parties, backed monetarily by the rich, have any real chance of propagating their beliefs and winning elections.
What democracy will look like exactly under socialism is yet to be determined as socialists are not in complete agreement about how to implement democracy. Some like Eat the Rich above advocate a centralized system while others (such as anarcho-communists) advocate a much more decentralized system in which various councils or regions are less obedient to any sort of centralized organ (if there even is one). Then there who are those like me who believe in some sort of centralized state that still recognizes individual councils or regions right to autonomy and self government.
