If borders did get eliminated, how would areas of land be named?
Would Islands still be called by their current names? (Like, Greenland, New Zealand)
And what would happen to countries like Germany?
would the land they once occupied still be called Germany, or changed to central Europe, or something completely different?

It's really not an issue. There is no way of knowing anyway but I don't see a problem with communities being named.

I don't think it matters much.
We have regions and areas now which have names and are not an administrative entity/state/nation.

I suppose we would just use the same names as we do now for stuff. I suppose some communities may want to get change their name, but that would be up to them.

In my opinion, islands and continents and general regions within them (i.e. the Outback or Bohemia) would have the same name, but there would be no "Germany" or "United States". Those names would only be a blatant reminder of the society we left behind. But in order for this to work, some regions would have to be renamed. Clearly the name "New England" would be inappropriate.

Depends entirely on whether you think that the nation will outlive the state. Personally I suspect they will - in which case a 'Germany' will continue to exist. If only because its easier to say than 'That Geographic Area Formally Known As Germany'
Nations are not, to paraphrase Metternich, merely geographical expressions

I think the inhabitants of each community would decide how to name them. It is pointless to ponder it now though, the situation will be handled at the time. The names of regions formerly known as countries would still have their old names though in my opinion.

I don't think it matters that much, to be honest.
Why would existing names have to be changed? Unless the inhabitants really want to change the name to something else...
We shall call them after the lands of Middle Earth. I call dibs on Mordor.

I think we should name everywhere after particular personal traits of myself, such as Charming, Intelligent, and Handsome.
But thats just my opinion.

Yhea else there would be huge confusion as rest assured if councils start talking about renaming a country it would go through dozens of naming with a few years before they settle on one, for example "It should be the Workers Democratic Region of (country)", "No that is stupid it should be called the Free Workers Region of (country)", "No, that is even worse, it should be the Socialist Democratic Region of Free Workers of (Country)".

To avoid confusion I don't think the names should be changed. Just because a place would have a particular name it still wouldn't be a nation in the sense that we know it. You could still have a region called Canada, or even break it up into each individual province. However, if the citizens of a particular region want to change the name they can. For example, I don't think very many people would want to keep the names that remind of an Imperialistic past.
Names wont change, people used to them, and trying to change that this place been england, or America or whatever for thousands of years now is the red land, or free spirit  or something is something would cause confusion.Naming dont matters, its for good understanding and helping us in moving, in knowing where we go etc.
Change lots of names would probably be against our own comfort. So no i dont think this would happen.

England will be called Airstrip One. Nah, but in all seriousness:
I don't think any major changes would occur, mostly for practical reasons most areas and places would likely be referred to as before. However, it's certainly possible that places names with, for instance, overtly religious etc connotations might get renamed on popular demand -- to disintegrate even the memory of oppression and ignorance.
Or like redstar2000 (http://en.wikipedia.org/wiki/User:Redstar2000) used to put it; 'Good bye San Francisco, hello Yerba Buena!'
